the first time i did it-nothing. i did the other 3 and then tried it one more time and it worked just now
I had no problems. The last time I tried to get a sample I had lots of problems. When I tried to use the website in English the offers disappeared. When I did everything in Spanish, no problem. I think now that I have a “cookie” indicating that I speak both languages I don’t have a problem using the site. This is just a guess…

The first time I tried it let me fill out the form then when I clicked-nothing. So I refreshed and filled it out again and it went through.
